Idiopathic Granulomatous Inflammation Presenting as a Malignant Tumor-like Mass in the Uterine Cervix: A Case Report

A granulomatous inflammation is a distinctive inflammatory response incited by various infective, autoimmune, toxic, allergic, and neoplastic conditions. It rarely involves the uterine cervix, and its appearance may mimic that of cervical cancer, complicating the diagnosis. Herein, we report a rare case of cervical granulomatous inflammation that led to the formation of a malignant tumor-like growth. An 88-year-old postmenopausal woman with abnormal vaginal bleeding presented with a visible mass in the uterine cervix. Radiological investigations revealed a suspiciously malignant lesion extending into the left parametrium and the lower vagina. However, histopathological examination of sequential biopsy samples from the lesion revealed only granulomatous inflammation, with no evidence of malignancy. We followed an expectant management, as the etiology remained unknown. At 6 months, the mass lesion was observed to have reduced in size. Our experience indicated that idiopathic cervical granulomatous inflammation can be simply monitored without any therapeutic intervention after the definitive exclusion of a malignancy using histopathological examination, even if its appearance is similar to that of cervical cancer.
